uhhh...it ain't that bad...i've seen ALOT worse in EMT textbooks. i dunno if it's real....like you should be able to see many more arteries and blood vessels in general for it to be real.....hmmm hard to say man.

one of the docs here would probably have a better idea.

I thought there would be alot more blood since the penis is basically all blood vessels. But the guy did wrap his penis really tight to constrict the blood flow I guess. Also, it seems like its not the first time the guy successfully cut off an appendage, if you look carefully, hes missing a finger as well.

I have to admit I've talked to a lot of people who are into body modification after I had mine (minor, nipple piercing), and fell into a group that included amputation lovers... self-sescribed as nullifactioners. These people really do exist. The pictures show he's already missing his index finger. I'm somewhat surprised that he went for his head with no other fingers missing... I'll assume he's missing other chunks of flesh and bone not shown.

He's had good advice, stopping the bloodflow in two places. I'm a little curious to know how he cauterized the large wound... fire or silver nitrate or something else.

do they do anything with the pieces?Well, depends why they're doing it. I can't cover all the reasons but genital removal is usually done:

1) Removing the primary link to manhood,
2) To stem the libido (symbolic and functional in terms of hormone regulation),
3) Aesthetics (removing this "bump" from the crotch looks nice),
4) Transgender, although this is not true nullifcation nor the best way to achieve it,
5) To stop pain (such as testicle pain),
6) The Hijras (http://en.wikipedia.org/wiki/Hijra_%28India%29) of India are an interesting case,
7) Fetish, which is usually part of a domination fetish in terms of dominating a partner so completely. In that case, some fetish fantasies involve cooking and consuming the pieces.

Other than that last case, I can't really imagine someone doing anything particularly interesting with it. It's biowaste and needs to be disposed of properly.

Since this guy is missing a finger, it's harder to figure out why he's doing it. Some psychologists link amputation to a type of OCD in which someone believes there are defects that need to be "corrected." I can't really see much difference from plastic surgery.
